COURSE REGISTRATION FORM

All courses have a 3 student minimum enrollment. If class minimum is not reached 24 hours prior to course, it will be cancelled and students will be notified via email and text.
Payment will be collected in person on the date of the course. Cash, Check/Money Order, Credit/Debit Cards, and Venmo are accepted
Family and Friends CPR can be taken any time a Heartsaver CPR/AED class is scheduled. Please make note of F & F while registering but choose a Heartsaver CPR date.
